Circle the City (CTC) is a nonprofit community health organization with a mission to “create and deliver innovative healthcare solutions that compassionately address the needs of men, women and children facing homelessness.” CTC’s system of homeless-specialty healthcare is one-of-a-kind in Arizona and is one of few similar models in the nation. Nearly 10,000 men, women and children experiencing varying degrees of homelessness will receive care in one of CTC’s many sites located throughout the greater Phoenix area.
Position Summary:The Patient Navigator works primarily within the integrated health care team with the goal of improving access to care and patient outcomes.

Qualifications:
Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors;
Experience in working with culturally diverse, chronically homeless, and medically needy individuals preferred;
Demonstrates excellent oral and written communication skills;
Bilingual skills – Spanish highly desirable;
Must demonstrate critical thinking, problem-solving, and organizational and time management skills;
Possess and maintain a valid Arizona driver’s license and has use of privately owned vehicle; and
Requires a current Arizona Department of Safety Clearance Card.
Physical and Mental Requirements
Demonstrated interest in working with an underserved population.
Position requires extended periods of sitting and standing.
Position requires regular bending and reaching, including transfer of patients.
Must be able to lift, carry and push 20 pounds on a regular basis.
Must be able to lift and carry up to 50 pounds on an occasional basis.
