CINQ-NY HEALTH HOME IPA LLC is Hiring a Clinical Pharmacist Near New York, NY

Overview

The Clinical Pharmacist will work directly with providers and healthcare professionals within the CinqCare network. They will be an integral member of the Value-Based Payment (VBP) team, using advanced data and analytics to increase quality metrics around medication adherence and disease management. As a member of the VBP team, the clinical pharmacist will also play a critical role in identifying pharmacy opportunities that impact the total cost of care. The clinical pharmacist will work with and report to the Chief Medical Officer to improve and expand telehealth and tele-pharmacy within and across the organization. Uses formulary knowledge to direct provider and patient education on cost and therapy optimization. The Clinical Pharmacist should embody CINQCare’s core values, including Imagination, Partnership, Forthrightness, Humility, Courage, and Relentlessness.

Primary Responsibilities

The Clinical Pharmacist will have the following responsibilities:

Primary Responsibilities & Duties

• Participate in medication review and reconciliation to ensure patient safety

• Develop pharmacy plans and protocols for common diseases such as hypertension, diabetes, heart failure, and other chronic conditions

• Medication Adherence: Works with local retail pharmacies and IPAs to achieve our goals with medication adherence

• Work with the VBP and Telehealth teams to create and implement strategies to improve medication adherence

• Present evidence-based guidelines, quality improvement, and savings opportunities to the CinqCare network

• Implement telemedicine protocols and provide patient education at office and telehealth visits

• Create an early-warning system to identify patients who are noncompliant with their medications

• Enhance the utilization of Surescripts within MEDENT and other EMR tools

• Any other duties as directed

• Deploy PDSA best practices across the CinqCare network to improve operational and quality metrics

  • Leadership: The Clinical Pharmacist will assist in the execution of the CINQCare Connect clinical model and pharmacy protocols by identifying pharmacy opportunities that impact the total cost of care, developing pharmacy-related plans across disease states to improve patient outcomes
  • Collaboration: The Clinical Pharmacist will work closely with other clinical departments to learn their needs, internalize their knowledge, to achieve the clinical objectives of CINQ Care Inc.
  • Knowledge: The Clinical Pharmacist will provide operational solutions and tailor learning opportunities to support the deployment of program interventions, use of CINQCare’s support tools and other data resources
  • Culture: The Clinical Pharmacist is accountable for creating a productive, collaborative, safe, and inclusive work environment as part of the larger Company.

Qualifications

The Clinical Pharmacist should have the following qualifications:

 Qualifications 

· Education. Pharm.D. degree

· Experience1-2 years of experience

· Entrepreneurial. CINQCare seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. This position is accountable for ensuring CINQCare is positioned to innovatively deliver on its mission and vision

· Communication. Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present complex concept models in an accessible manner to CINQCare’s team, investors, partners, and other stakeholders.

· Relationships. Ability to build and effectively manage relationships with business leaders and external constituents; and

· Culture. Good judgment, impeccable ethics, and a strong collaborator; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Company.
