CICOA is Hiring a CHOICE Navigator Near Indianapolis, IN

Assists the department with individual-related needs by providing communication and follow-up to enhance quality of the CHOICE/waiver application and transition into waiver services. Works directly with the individual, family, care managers, and providers with the intention of creating an exceptional experience for individuals served by CICOA.

Essential Duties and Responsibilities
include the following. Other duties may be assigned.
  • Assist and monitor Medicaid application, eligibility, and approval for the individuals served at CICOA.
  • Support the client through the initial application process through confirmation of in-home services and transition to appropriate care management team.
  • Assist the client with the Medicaid application process, which may include the gathering and submission of required documentation,
  • Travel to the client’s home, office, and FSSA office to gather and submit supporting documents as needed which may include travel in all of CICOA’s 8-county service area.
  • Attend interviews either in person or via phone with the Division of Family Resources Office as required.
  • Assess the needs and eligibility for funding streams in the community to provide linkage to needed services for clients including assistance with Medicaid, SNAP benefits, Energy Assistance, and other community resources.
  • Assist with educating family, supports, and individual on Medicaid and Medicare.
  • Serve as a client’s advocate.
  • Perform other duties as assigned.

Education and/or Experience
High school diploma or equivalent required; computer experience required; email, Microsoft Office suite and internet. Two years relevant experience in a medical, care management or Medicaid/Medicare setting preferred. Associates Degree in Human Services, Psychology, Nursing, or related field preferred. Healthcare experience preferred. Relatable experience working with older adults highly preferred. Experience working with state agencies and/or with Medicaid approval process preferred.
