What are the responsibilities and job description for the Medical Assistant position at Chronica?
Talented professional for community outreach driving patient education and vital sign monitoring. You will represent the Chronica and work with the management team to develop and implement initiatives that increase visibility within the physician practice or other facets of the healthcare continuum.

Responsibilities:
- Maintaining an education calendar of patients scheduled for onboarding (remote patient monitoring)
- Present as a SME and use knowledge and authority to drive educational experience with office based medical professionals.
- Prepare a daily schedule of patients using master-list provided by Chroinca.
- Preparing and maintenance of accurate records and reports on the goals enrollment. Enroll 6 to 8 scheduled appointments daily.
- Communicate weekly with team using tele-features of Hifinite platform.
- Provide list daily of newly onboarded patient lives and any important characteristics.
Requirements:
- Certificate or pending in communications, marketing, healthcare, business, or related fields.
- Preferred experience within medical delivery system.
- Outstanding written communication and verbal skills.
- Strong experience preparing reports.
- Friendly, enthusiastic, and positive attitude.
- Strong knowledge of healthcare and other basic electronic platforms.
- Detail-orientated with the ability to manage multiple projects at same time.
Note: the position is office based and mobile/homecare enrollment & education
