United Recovery Project is seeking a professional, dedicated, and compassionate Case Manager to join our outstanding Clinical Team. Candidate must have a thorough understanding of mental health disorders, and have the skills and compassion necessary to work in this environment. Core Responsibilities of the Case Manager:

• Coordination of case management services.
• Completes assessments, insurance and FMLA documents, and provides discharge and aftercare services.
• Communicates with law enforcement and probation, employers and government agencies on behalf of the patient.
• Builds relationships with community partners to provide clients with referral and linkage to community resources.
• Acts as a liaison between the multidisciplinary team and other departments, families and other outside agencies to advocate for the patient and remove barriers to care.
• Assists with ACA (Against Clinical Advice) blocking and patient management.
• Participates in case staffing's and reports concerns.
• Reporting Suicidality/Homicide of a patient based to their job position.

Pay Range $40,000 to $62,000 annually commensurate with experience.

Requirements
Qualifications of the Case Manager:

• Valid and active Driver License is required.
• Computer literacy required.
• A minimum of 2-year experience in Mental Health required.
• A minimum of 1-year experience in Case Management prefered.
• Highschool Diploma or GED required.
• KIPU EMR knowledge preferred.
• Current CPR certification and First Aid preferred.
• Ability to work overtime, weekends, holidays, and flexible hours.
• Applicants are required to successfully clear a Level 2 Background check.
