Job Summary

This position is responsible for assisting in the intake of all children, youth, and families. S/he will oversee the initial processing of all referrals, taking responsibility for gathering and entering all related data. Additionally, this person will also assist the Senior Director with recruitment and other related community outreach tasks and events.

Responsibilities and Duties

  • Receive program referrals, either from DCYF or community 
  • Screen referrals using eligibility criteria to determine eligibility
  • When necessary, facilitate intake meetings within contract timelines. This may include urgent and emergency referrals from DCYF 
  • Enter all referral demographics into our electronic health record, MyEvolv, and all referral demographics and referral documents into the and Rhode Island Family Information System (RIFIS)
  • Following intake, open case in MyEvolv and open case and assign (with guidance Senior Director) in RIFIS 
  • Following discharge, close case in MyEvolv and RIFIS with correct closing descriptors
  • As needed, audit client records at discharge from program
  • Following discharge, submit all closed files to records department 
  • Assist Senior Director with monthly census related tasks such as ensuring all medical ID numbers have been obtained and put in RIFIS, ensuring that all discharged cases are completed and closed in RIFIS by the 1st of each month, ensuring that all records are obtained upon case closing 
  • As needed, assist Senior Director with requests to DCYF to service families past 180 days and billing requests to DCYF
  • Maintain ongoing communication with DCYF and various community providers as well as co-location as deemed needed and appropriate
  • Work in conjunction with Senior Director to organize, attend, and present at community engagement events and in efforts to promote the program within the community 
  • Compose correspondence to variety of internal and external audiences, primarily DCYF and Program Managers/Directors/VP’s
  • Maintain database for all incoming referrals and other data necessary for Dashboard and other tracking tools
  • Ensure that all work is accomplished in a safe manner. This includes the safety of the employee as well as others who are using the facility

Qualifications and Skills

  • Bachelor's degree in social services or related field 
  • Experience in a human services non-profit organization preferred 
  • Comfortable with technology
