Are you mission driven? So are we! Chestnut Health Systems seeks an LPN to work in its residential behavioral health programs. Provide for patients’ physical health needs as they seek and maintain recovery, embracing Chestnut’s whole-person approach to care. Full-time position, Monday – Friday, noon until 8:30 p.m. No weekends, no holidays, and eligible for shift differential!

Responsibilities

Conduct physical health screenings to ensure clients are medically appropriate for admission. Assess clients for acute physical health issues. Provide first aid as indicated. Make physician referrals. Coordinate care among prescribers, clients, laboratories, PCPs, other health care providers, and pharmacies and staff to ensure continuity of care. Transport residential patients to medical and other appointments.

Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness.

Qualifications

High school diploma or equivalent. Active Illinois LPN license. CPR certification (or obtain certification within 90 days of hire). Basic computer skills including MS Word, Outlook, e-prescribing, and an electronic medical record (EMR). Valid driver’s license, private automobile insurance, and ability to be insured.
