Minimum Qualifications/Requirements: Bachelor’s Degree in a human service field. Salary is commensurate with degree and experience.

Summary Description of Position: Case Management consists of services that help clients gain access to needed medical, social, educational, therapeutic and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. Case Managements includes: (1) assessment of the eligible client to determine service needs; (2) development of an individualized integrated care plan; (3) referral and related activities to help the client obtain needed services; (4) monitoring and follow-up; and (5) evaluation.

Case ManagerResponsibilities:

· Ensure safety of clients and fellow colleagues as a number one priority at all times.

· Protect clients and fellow colleague’s information in regards to confidentiality.

· Maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.

· Conduct admission of clients into the program and ensure clients are educated about the program rules and regulations.

· Maintain an active caseload providing interventions as needed within area of expertise and limits of credentials; assures procurement of additional services as needed.

· Interview residents, referral resources, and significant others to obtain needed information to ensure accurate treatment planning and discharge planning.

· Complete intake assessment for clients with complete and accurate medical, psychiatric, and addiction history.

· Complete a biopsychosocial assessment to include history, basic personality, and to identify problem areas for residents.

· Conduct random urine drug screens and breathalyzer tests.

· Provide crisis management for residents; makes linkages for interventions as appropriate.

· Acquire and provide the clinical team detailed information regarding resident to establish the foundation for the Clinical Treatment Plan.

· Assist in development, implementation and revision of individual treatment plans; assures that services provided are specified in the Treatment Plan and monitors progress toward treatment goals.

· Provide residents individual and group Activities of Daily Living (ADL) training.

· Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.

· Conduct client education on addiction, recovery, and/or other areas as needed for provision of resident care.

· Participate in individual and group therapies as assigned, as well as complete clinical documentation for the resident record.

· Maintain detailed and timely reports, records and notes of resident’s progress. Maintains client and program records in accordance with applicable standards and regulations.

· Participate in interdisciplinary treatment planning conferences, resident community meetings, clinical meetings and program meetings.

· Interpret clinical findings and medical recommendations to client, family referral sources and as indicated.

· Maintain contact with all referral sources and participate in referral development activities as needed.

· Attend meetings as appropriate and meet regularly with supervisor to exchange pertinent information and receive supervision.

· Transport of clients in company vans to attend Partial Groups as assigned. At times, willingness to use reliable personal transportation in work capacity.

· Formulate and coordinate Recovery Enhancement planning, and complete clinical discharge summary.

· Maintain clear and effective notes about the client’s progress and communicate effectively with staff about any needs or concerns regarding the residents.

· Maintain clinical licensure/certification and current clinical competence and uphold professional code of ethics.

· Participates in continuing education activities, remaining knowledgeable in area(s) of expertise.

· Maintain positive and clean work environment for clients and clinical team at all times.

· Cooperate with other employees, including management in a respectful, helpful manner.

· Proactively seek guidance and counsel on how to respond to certain events or circumstances.

· Respond to emergencies in accordance with first aid and CPR training, and good judgment.

· Abstain from alcohol, nicotine, illicit drug, and/or other substance use while on duty, on call or on Employer’s premises.

· Other duties as Employer may assign from time to time.

Certificates, Licenses, Registration:

  • Valid driver's license in accordance with NC DOT.
  • Maintain current American Red Cross CPR/1st Aid certification & Seizure Management
  • Maintain current Least Restrictive Interventions
  • OTHER REQUIRED TRAININGS VIA NCDHHS

Job Type: Full-time

Pay: $15.00 - $18.00 per hour
