Cerebral Palsy of Westchester is Hiring a Quality Assurance Coordinator Near Brook, NY

SUMMARY:

  • Responsible for ensuring that quality services are provided in all realms to all programs under the auspices of OPWDD, SED, OCFS, NYS Justice Center, and Westchester County DOH.

ESSENTIAL DUTIES AND RESPONSIBILITES:

  • Overall management of all incidents reported or discovered which involve the individuals who receive services from the Agency. This includes but is not limited to making notifications, completing reports, collaborating with the Program Director, entry into related databases, presenting findings, conclusions, and recommendations, and oversight of protections and corrective actions.
  • Conduct formal investigations into alleged incidents involving the individuals who receive services from the Agency.
  • Conduct orientation and the required annual training that pertains to Quality Assurance.
  • Attend trainings for professional development related to the position.
  • Chair and attend committee meetings as directed, such as: Incident Review, Special Review, Informed Consent, Safety, etc.
  • Provide support and direction to Program Directors and Managers in gathering information that is pertinent to incident reporting and management and other areas related to Quality Assurance (i.e., Safety, Fire Safety, Emergency Preparedness).
  • Manage on-call responsibilities as assigned to address emergency situations within the Agency.
  • Manage internal Quality Assurance audits, observations, and reviews within the Agency. This will include traveling to program sites and performing unannounced visits to departments and programs to ensure compliance with relevant regulations, agency policies, procedures, and practices during the provision of services.
  • Manage external Quality Assurance audits and reviews within the Agency. This will include traveling to program sites to interface with and provide requested support to the program and audit team throughout the survey for all programs.
  • Act as a liaison as needed between individuals, family members, external entities, and the agency relative to incident management and safety.
  • Manage the annual satisfaction surveys for all programs, report the outcomes, and make recommendations for quality improvements based on feedback received.
  • Generate an annual trend report for all programs, and a monthly report of all responsibilities managed to the Division Director of Quality Assurance, Compliance, and Training.
  • Perform other duties as directed by the Division Director of Quality Assurance, Compliance, and Training.

JOB REQUIREMENTS:

  • Knowledge of the program planning process and a good understanding of human dynamics.
  • Knowledge of regulatory requirements for various programs within the Agency.
  • A strong ability to work efficiently, independently and to apply sound judgment in work situations.
  • Proficient in the use of Microsoft Word, Excel, Outlook (email), and the ability to complete and file assigned paperwork.
  • Strong presentation skills in oral and written form.
  • A valid driver's license and a reliable vehicle.

EDUCATION AND/OR EXPERIENCE:

  • Bachelor’s Degree (Master’s Degree preferred) in a related field of study. Consideration will be given to a candidate who has extensive experience in this area who does not possess a degree of higher education.
  • A minimum of 3 years work experience in Quality Assurance, investigator, or supervisory capacity.
  • Work experience in the Human Services field, preferably with the developmentally challenged and/or dual diagnosed population within a residential and/or school setting.

COMMUNICATION:

  • Effective and professional oral and written communication skills.
  • Ensure that established procedures for notifying appropriate individuals of significant events, including but not limited to emergencies, incidents, maintenance, or environmental safety needs, are followed.
  • Ability to communicate effectively with residents, families, and/or advocates, co-workers, and supervisors.
  • Communicate in a manner that is respectful of the diversity of the workplace environment.

TIME MANAGEMENT:

  • Ensure excellent attendance and punctuality.
  • Flexibility to allow for completion of required trainings, including but not limited to orientation, staff meetings, annual trainings etc.
  • Sufficient flexibility in regard to assignments and schedule.
  • Ability to meet deadlines and expectations with regard to projects.

WORKING CONDITIONS:

  • Environmental Demands: Ability to meet the environmental demands of the position, such as recreational and leisure activities, hospitalizations/emergency room visits, medical professional appointments, religious services and other functions or events as directed.
  • Physical Demands: Ability to meet the physical demands of the position such as lifting, bending, reaching, stooping, kneeling, climbing and balancing.

  • Demonstrate effective and professional oral and written skills using the English language.

Job Type: Full-time

Pay: $56,000.00 - $60,000.00 per year
