Job Description: The Wellness Coach will provide intensive linkage, referral and advocacy services to a small caseload of adults and children with mental illness, including substance abuse. Job requires home visits (mileage reimbursed) and documentation of all work with consumers. Must be able to work within a team based approach. Laptop, air card and cell phone provided. Requirements: Bachelor’s degree in social work, psychology, sociology, or education preferred. Case management experience preferred.
