Job Description: Nurse (RN or LPN) sought for a position at Centerstone Mental Health outpatient clinic in the Melrose area. Job duties include providing face to face care, observe patients’ health, assist a team of prescribers, communicate instructions to patients regarding medication, phlebotomy, administration of long acting injectable medications, accuracy in measuring and documenting vitals, liaison between prescribers and other disciplines in the facility, participate in weekly treatment team meetings and other duties as required. No weekend or on call required. Occasional coverage may be asked for other local clinics. Job Qualifications: Graduation from an accredited nursing program is required Minimum of six (6) months of experience in psychiatric or equivalent mental health related nursing experience is preferred Must have the ability to work cooperatively with all staff and physicians and function as a valuable member of the treatment team TN LPN -or- RN required Essential Duties and Responsibilities: Performs comprehensive psychiatric /mental health assessments, evaluates mental status and refers to other treatment providers when necessary Monitors changes in health status, including but not limited to vital signs, weights, nutritional status and disease states Provides assigned residential nursing services as directed Administers and monitors medications as ordered. Monitors medication effectiveness, side effects and compliance. May perform assessments for any side effects/EPS Addresses medication issues with consumer when necessary. Provides proper education and guidance as necessary Provides healthcare education to clients and/or families or staff as assigned Works in collaboration with other members of the treatment team and in collaboration with external health care providers as necessary Regular and Reliable attendance is required Key Performance Indicators Meets billable hour productivity standards Completes clinical documentation within organization’s policy standards Maintains all mandatory trainings up to date Other Duties and Responsibilities: All other duties as assigned Time Type: Full time Delivering care that changes people's lives starts with our employees.
