Job Description: SUMMARY OF POSITION Provides person-centered assistance to clients in the development of life skills and personal goals. Meets one-on-one and in group settings to support development of skills around all types of life skills, to include but not limited to, parenting skills such as self-management of medication needs, money management, cooking, cleaning, transportation, financial assistant and securing appropriate entitlements. Supports development of life skills for children, including social skills, impulse control, study skills, hygiene, and independent living skills. Utilizes evidence-based practice to support clients’ needs and goal attainment. ESSENTIAL DUTIES & RESPONSIBILITIES Coordinates client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client’s clinical record, including but not limited to treatment plan, progress notes, reporting and correspondence with external agents, and remain in compliance standards. Teaches and trains clients in activities of daily living, in a group and individual settings, complying with clinical documentation requirements, and provides services according to organizational guidelines or protocols. After evaluation and assessment, connects client with community support systems as determined to be appropriate, and maintains regular communications with schools, OFC’s and other family service providers that the client and their family is involved with or receiving services from. Consistently exercises discretion and judgment to analyze, interpret, make decisions and decide what actions are necessary based on the varying facts and circumstances of each individual case. Provides telephone and on-site crisis intervention/prevention and follow-up to clients and their families. KNOWLEDGE, SKILLS & ABILITIES Effectively communicate via written, verbal, in person and virtual methods. Customer service skills. Time management skills. Analytical skills. Knowledge of community resources. QUALIFICATIONS Education Level Associates degree. Bachelor’s degree preferred. Two years of experience or two years of college coursework in related field may be substituted for degree requirement. Years of Experience Two years professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice behavioral health or related setting preferred.
