Job Description: Health Coach – Adult & Family Services The Health Coach will provide individual supportive coaching to clients with chronic mental illness and substance use disorders, focusing on improving their physical health and mental health status. The Health Coach will provide life skills, case management, and structured interventions to assist clients in gaining access to resources and developing skills to improve both their physical and mental health, quality of life, and self-management of symptoms. This will be done by focusing on areas such as health hygiene, lifestyle habits, nutrition, reducing sedentary behavior, and healthcare navigation. The Health Coach will be expected to engage with clients in the community and over telemed, and have reliable transportation. Job Qualifications: Bachelor’s degree from a four-year college or university is preferred. Experience working and/or volunteering within the behavioral health, criminal justice, or social services field can be considered in place of degree. Excellent verbal skills, ability to communicate effectively with a wide variety of persons – i.e. clients, clinical staff, healthcare providers, etc. Driver’s license and satisfactory driving record required.
