CCSWW is Hiring a Lead Case Manager Near Tacoma, WA

Overview

$22.11 - $24.88 HR/DOE

The Lead Case Manager is a member of the Supportive Services Team for Homeless Adult Services (HAS), a division of Catholic Community Services. The goal of this position is to assist clients of the Nativity House Overnight Shelter and/or Cavanaugh Stability Site, exit shelter to permanent housing. This position supports the Program Supervisor in problem solving, resource management, reviewing and auditing case files for ongoing quality assurance and training purposes. The Lead HAS case manager works closely with the Supervisor and the community at large, he/she also provides support as requested with the overall objective to provide outstanding case management to all individuals in the Homeless Adult Services programs. This position also maintains a caseload which includes developing and implementing client-defined, goal-oriented Housing Stability Plans and supports and helps the clients be accountable to the HSP created.

This position also works closely with the other members of the Mental Health Team and Shelter Staff to assist guests in reaching the goal of housing.

Responsibilities

Assists with training and oversight to Case Management teams while promoting a cooperative team environment. Support and teach HAS teammates to ensure ongoing growth in skill level and experience working with issues impacting homeless adults and high-quality case notes and case files.

Employee accomplishes this responsibility by completing the following tasks:

  • Assists with scheduling and performing with fellow case manages, periodic review of case files, case notes and other pertinent information for quality assurance, compliance with funding contract requirements.
  • Collaborates with Program Supervisor concerning critical program information, community resources, case management training needs and other information to assist in managing HAS Supportive Services program.
  • Assists the Program Supervisor with the development of case management standards, guidelines and paperwork to assure high quality care management to clients.
  • Actively participates in weekly case management meetings.
  • HMIS participation: participates in all required HMIS trainings; collaborates with CCS Data Administrator concerning data quality needs specific to Coordinated Entry referrals and assignments. 
  • Assists the Supervisor in tracking and assigning clients to HAS Case Managers.
  • Budget management: actively participates in spending down appropriate funds, works with supervisor and manager to stay within budget limits and spending needs.
  • Serves as a mentor to fellow staff members, available for consultation on client cases as needed and reports pertinent information to Supervisor.

Assess Housing needs using Motivational Interviewing and a Progressive Engagement Approach.

  • Build a relationship of trust with the clients that can be used to support the client in finding housing.
  • Listen to client’s story and summarize back to them.
  • Identify immediate housing barriers as well as strengths and potential assets and help households brainstorm creative options for alternate housing arrangements using Motivational Interviewing Techniques.
  • Conduct Brief Screening to identify Housing, Mental Health, and Substance Use Disorder needs.

Develop and Initiate Housing Stability Plan: Assess barriers to housing as well as the client’s strengths and needs, and develop a strength-based and client-centered plan for housing and housing retention. Work collaboratively with households to determine steps needed to secure housing.

  • Document action plan for short-term intervention to resolve housing crisis, use of community and natural resources, and family strengths to create a path to permanent housing stability.
  • Meet with client to develop a strength-based and client-centered plan for obtaining and retaining housing. This will include:
  • Develop plans for addressing barriers such as previous evictions or convictions.
  • Utilize client strengths to develop and implement plans.
  • Work with client to develop realistic plans to handle difficulties in the future. This may include disagreements with landlords or other tenants, difficulties with friends who visit, and maintaining recovery.
  • Develop with Client specific measurable goals and action steps to achieve the goal of gaining permanent housing as well as other goals the client may have.
  • Provide case management to HAS clients to assist clients in acquiring housing.
  • Meet with the client to review progress on identified action steps, possible options, and assisting the client in taking additional appropriate action steps to further plan goals.
  • Help the client establish next action steps that are appropriate to the client’s level of ability function, and independence.
  • Work with client to address barriers to housing to include evictions and convictions.
  • Work compassionately with clients to overcome challenges in motivation as well as the impacts of trauma and years of homelessness and poverty.
  • Reviewing the housing stability plan with the client and update as appropriate.
  • Provide records and reports. Keep accurate and timely case management notes.
  • Participate in weekly case management meetings and provide case summaries and client status information.
  • Keep records organized and confidential.
  • Prepare weekly report to include statistical information.
  • Attend meetings as necessary. Attend weekly Supportive Services and Mental Health Team Meetings meeting.
  • Attend other meetings as required by the Clinical Supervisor.

Coordinated Entry

  • Provides screening and intake of people seeking homeless services, utilizing assessment and prioritization tools as well as homeless diversion strategies.
  • Follow established schedule and communicate any need to deviate from schedule.
  • Respond promptly to emergency referrals.

Work Readiness

Weekly class - that provides individuals experiencing homelessness with job leads, weekly

 workshops for resume assistance, goal setting, Job search monitoring for Housing Stability

 goals, professional work attire, community resources, networking with employers and hiring

 events, weekly updated curriculum, and referrals to worker retraining options.

Community Outreach- Networking with organizations such as Goodwill, Work Source, Department of Corrections, Career Path Services, JBLM, Bates Technical College etc. to connect those seeking employment with additional supports during class time.

Mock Interview Sessions- Bi-monthly event that is open to the community to provide job

seekers an opportunity to perfect their interview skills in a professional setting. This involves

coordination of staff, interviewees, guest speakers, interview materials, community outreach,

and space availability.

Donations- Gather, sort, and distribute community donations of work attire.

HIPPA

  • Responsible for upholding a culture of privacy and security in highly confidential work environment (HIPAA covered entity service area).
  • Complies with all CCSWW policies and procedures that involve access to and safeguarding of client Protected Health Information (all forms).

Other duties as assigned.

Qualifications

Minimum Qualifications

  • AA in a human or social services field, or combination of college and relevant experience.
  • Ability and willingness to work in a team environment and promote a positive team spirit.
  • Excellent inter-personnel skills including excellent oral and written communication skills
  • Excellent organizational and time management skills
  • Strong computer skills including Microsoft Word and database experience
  • Ability to work with a diverse spectrum of high-need individuals
  • Applicant must successfully pass required background checks prior to an offer of employment.
  • Ability to work independently and as a member of a team.
  • Proof of negative TB test within past 12 months.
  • Must have reliable transportation, valid driver’s license and automobile insurance.
  • Must have an acceptable driving record per CCS’ driving policy. (required to provide current driving abstract)
  • Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations
  • Support and contribute to a creative, collaborative and respectful environment that promotes teamwork

Preferred Qualifications

  • BA/BS Degree in Human Service field
  • Experience in case management
  • Knowledge of Pierce County social services and other community resources
  • Experience with transitional housing, landlord/tenant law and other housing issues
  • Bilingual skills (English-Spanish)
