ABOUT CASCADE:

Cascade Community Healthcare (CCH) has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Our mission is to provide our clients with compassionate and encouraging counseling leading to a renewed sense of hope and the ability to make positive changes and live more satisfying lives. Cascade currently has six treatment locations in Lewis County.

We are looking for a Care Coordinator to join our Adult services team! This position provides coordination between community members and Cascade Community Healthcare (CCH) clients who have been identified as needing assistance with accessing and utilizing the health care/behavioral health care system. You will work collaboratively with advocates, clinicians, and peer supports to provide support and assistance.

Cascade Community Healthcare does not require the COVID-19 vaccination as a condition of employment.

PAY SCALE:

$19.50 - $28.41 DOE

WHAT YOU NEED TO GET THE JOB DONE:

  • WA State Certified Medical Assistant credential required.
  • Agency Affiliated Counselor (AAC) credential required.
  • Must be WA State Peer Certified Counselor or must obtain WA State Peer Certification within 90-days of employment.
  • Bilingual preferred (English: Spanish).
  • Ability to interact with and engage clients effectively.
  • Must be able to pass WA State Patrol background check. A national background check may be required if haven’t lived in WA state consecutively the last three years.
  • Must be able to represent the agency in a positive and professional manner.
  • Must be able to communicate clearly, both verbally and in writing.
  • Must be able to enter information into a computer network system.
  • Must comply with agency policies and procedures. 

WHAT YOU'LL DO:

  • The employee must be able to accomplish the following tasks with or without accommodations:
    1. Initiate contact with clients and helps with accessing services as needed.
    2. Utilize various reports to identify and track clients who haven’t followed through with referrals.
    3. Coordinate with Community Providers and CCH as needed to engage clients.
    1. Coordinate client data and processing procedures.
    1. Coordinates with Community Providers and CCH personnel to ensure effective client care.
    2. Review symptoms with clients assigned to team receiving medication management services at CCH monthly. Report findings to nursing/medical department.
    3. Review treatment plan with clients, coordinate with treatment team if findings show that client is needing additional support, change in frequency of appointments.
    4. Maintains, records, and reports in accordance with agency policies and procedures.
    5. Participates in supervision, consultation, and training at the direction of the Program Manager.
    6. Meets direct service productivity requirements as assigned by the program manager/director.
    Peer Services:
    1. Draw on common experiences as a peer to validate clients' experience and to provide guidance and encouragement for clients to take responsibility and participate in their recovery. Promote hope and empowerment designed to help clients attain recovery goals and maintain the highest possible independence level, even during periods of instability.
    1. Act as an interpreter to help non-mental health client team members better understand and empathize with each client’s unique and subjective experience and perceptions.
    2. Provide expertise and consultation from a mental health consumer perspective to the entire team concerning client’s experiences on symptoms of mental illness, the effects and side effects of medications, clients’ responses to opinions of treatment and clients’ experience of recovery.
    3. Collaborate with the team to promote a team culture in which each client’s point of view, experiences, and preferences are recognized, understood, and respected, and in which client self-determination and decision making in treatment planning are maximized and supported.

  • Medical benefits will be effective the first of the month following 60 days of employment. Must be at least a 0.75 FTE (30 hours/week) to be eligible for medical benefits. Must be at least a 0.5 FTE (20 hours per week) to be eligible for dental, vision, Teladoc, and Life and AD&D.
