Description

The primary purpose of this position is to oversee the provision of Family Foster Care & Therapeutic Foster Care Level I, II, & III services to consumers. The clinical supervision and training of Level I, II, & III providers, monitoring for appropriate service delivery, ensuring outcome-based services, and monitoring for compliance with service requirements . This process is facilitated by using the Together Facing the Challenge and CTS Inc model of care for consumers up to the age of 21.

 Responsibilities and duties:

• Recruitment and development of all program referrals
• Provide timely outreach to referrals for Foster Care parents, and maintain a database of referrals, recruitment efforts and outcomes.
• Follow-up on referral sources and collaborative efforts to promote CTS HEALTH, INC. Foster Care program.
• Oversee the coordination and assignment all referrals to clinicians for initial clinical assessments
• Monitor and report any concerning outcomes to your supervisor for all standard agency assessment documentation timeframes
• Ensure Client and Foster Parent rights are being followed.
• Provide case management to ensure proper assessments are conducted for each consumer and that the consumer is at the proper level of care based on assessments and behaviors.
• Complete SAR requests for level I, II, & III therapeutic consumer authorizations by submitting through EHR.
• Maintain 100% Authorizations with no gaps in service for all Level I, II, & III therapeutic consumers.
• Provide 24 hour on call support for crisis calls and critical incidents.
• Apply clinical and child development knowledge to develop IPOCS that meet the needs of the consumer.
• Provide clinical case management to therapeutic families and children who are aggressive, emotionally disabled, and in need of therapeutic home setting.
• Assure all Comprehensive Case plans are developed with age and developmentally appropriate goals and distributed to all meeting participants.
• Coordinate the approved services with all treatment team members (i.e., consumer, family, DSS, DJJ, school systems and other health care professionals).
• Attend Court Hearings, Family Group Conferences, Guardian ad Litem meetings, Foster Care Review Board meetings, Education meetings, Mental Health appointments, and any other Interdisciplinary meetings scheduled either by CTS, DJJ, SCDSS or another involved agency to determine best care for the consumer.
• This position will be expected to meet with their direct supervisor at least one time a month for supervision.
• Other duties identified by supervisor deemed necessary to improve the quality and continuity in services provided
