Caring Friends Home Care (a HouseWorks, LLC company)

The primary purpose of this position is to develop a comprehensive care plan to meet the physical and emotional needs of the clients and the client’s family. Care plans are mandatory per the Department of Health guidelines. Additionally, the Home Care Supervisor is responsible for completing weekly Medication Management visits in the client’s home, New Client Assessments and the Start of Care Conference. The Home Care Supervisor also completes all mandatory caregiver competencies for new staff and annually as per the DOH.As team leader you direct and provide support to all other team clients. This position collaborates with other disciplines as necessary.

Duties and Responsibilities

1. Administrative Functions
   • Completes weekly or bi weekly Medication Management
   • Completes initial assessments and annual assessments
   • Enters Initial Care Plan and Annual Care Plan into HCIT
   • Responsible for updating Care Plans when there is a change in condition
   • Supervisors all caregivers and performs all related disciplinary actions
   • Maintains client records
   • Reports any change in client condition to PCP and client POA as directed
   • Investigates any accidents or incidents immediately in accordance with DOH policies
   • Investigates any client complaint or grievance and notifies Executive Director according to DOH policies
   • Notifies Executive Director if unable to report to a scheduled assignment or if unable to complete assignments (i.e. care plan) in allotted time
   • Assists Scheduling as needed
2. Nursing Functions
   • Assesses client in accordance with DOH policies
   • Provides basic nursing care in accordance with RN scope of practice.
   • Assesses client’s physical and emotional state as needed in accordance with DOH policies
   • Does comprehensive chart reviews as needed
   • Communicates with physicians and carries out collaborative plan of care for clients
   • Assumes the authority, responsibility, and accountability of directing the client’s care
   • Completes necessary assessment tools
   • Writes and implements comprehensive care plan
   • Provides medication reviews and communicates concerns to client and physicians
   • Trains and monitors all clients of the care team
   • Coordinates, as necessary, family meetings, Start of Care Conferences, Assessments and Staff Competencies
   • Identifies community resources and communicates with outside agencies when necessary to meet the needs of the client
   • In the absence of support staff, the team leader is able and willing to perform any of the duties in order to meet client needs
   • Works with administrators and nursing in facilities where Caring Friends provides care
3. Staff Development Functions

• Responsible for conducting all required DOH training for caregivers both upon hire as well as annual competencies
• Responsible for entering assessments and care plans into HCIT and updating them should there be a change in condition or annually per DOH
• Self-educates and formally stays abreast of home care standards and regulations of DOH
• Researches and implements interventions that are evidenced based to improve the care and quality of life of clients
• Consults with Executive Director in development of new programs
• Develops and updates employee orientation as needed

Specific Qualifications

• Education: Diploma from an accredited school of nursing (RN, LPN, PRN).

• Experience: 1-2 years of clinical experience in gerontology, home care, rehabilitation or acute care.

Specific Requirements

• Must be able to read, write and speak the English language.
• Must possess the ability to make independent decisions when situations warrant such actions.
• Must possess the ability to deal tactfully with clients, families and other Caring Friends staff.
• Must have patience, tact, a cheerful disposition as well as an ability to handle difficult clients.

Physical Requirements

• Must be able to cope with the physical and mental stress of the position.
• Must be able to see and hear or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.
• Must meet the general health requirements set forth by the policies of Caring Friends.
• Must be able to relate to and work with the elderly, disabled, emotionally upset clients.
• Must be able to lift (50 lb. limit)
• Must be able to transfer residents using mechanical devices.
