Job Summary:

Care Managers are responsible for working with Intervention Associates’ clients and referral sources to meet the unique needs and circumstances of the individual. Care Managers work with people of all ages and needs, including people with disabilities to those aging in place. Care management services are wide-ranging and can include, but are not limited to, comprehensive needs assessments, coordination of services, arrangements for in-home care, interface with insurance carriers, referrals to Medicare and Medicaid and all types of referrals, (e.g., attorneys, financial planners and others). Care Managers work from their home and in the field and can manage their own schedules.

Essential Duties and Responsibilities:

• Respond to referral from Intake/Admissions Coordinator.
• Perform comprehensive biopsychosocial assessment, develop care management plan in collaboration with client/family and provide care management services as appropriate.
• Have familiarity of public benefits and ability to assist clients to obtain them.
• Assist families respond to crisis situations.
• Provide assistance to the Executive Director in promoting Intervention Associates to existing and potential referral sources.
• Demonstrate proficiency in data entry and comply with all date entry requirements.
• Document all care management activities in electronic health record, generate reports and ensure current client data.
• Maintain confidentiality of client information and comply with HIPPA regulations and standards, as required.
• Provide on-call coverage during evenings, weekends, and holidays.
• Serve on committees/work groups as appropriate or requested.
• Understand and comply with all applicable policies, procedures, and standards.
• Perform other duties as assigned

Qualifications and Skills: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Master’s degree in Social Work or other Human Service or Health Care discipline such as nursing, physical or occupational therapy
  • A minimum of 3 years of care management experience working children, adults and older adults.
  • Must maintain credentials and state licensure as appropriate and required.
  • Strong verbal and written communication skills, including the ability to communicate effectively with people from diverse backgrounds and language skills.
  • A proactive, customer service orientation, with the ability to collaborate effectively with internal and external stake - holders.

Physical Demands: 

  • Requires occasional standing, sitting, walking; using hands to finger, handle, or feel objects, tools or controls; reaching with hands and arms; climbing stairs; hearing; talking

Work Environment:

  • Travel to and from client’s home
