How You'll Contribute: 

As a member of the interdisciplinary team, and reporting to a Clinical or Hospice House Manager, the Licensed Practical Nurse performs selected nursing care functions under the direction of the Registered Nurse, in order to maximize the comfort and health of patients and families. 

Responsible for providing supportive nursing care in accordance with the interdisciplinary plan of care and Care Dimensions procedures.

What You'll Bring:

• Graduate of state approved School of Practical Nursing accredited by the National League for Nursing and National Association of Practical Nursing.
• Currently licensed as a Licensed Practical Nurse in Massachusetts.
• Previous experience in a home health, long term care, hospice or hospital setting.
• Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
• Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
• Field staff LPNs, must be a licensed driver with a dependable automobile that is insured in accordance with state and/or organization requirements and in good working
