**MUST BE LICENSED AND RESIDE IN THE STATE OF ARIZONA** 

Basic Function: Responsible for the diagnosis, treatment, and maintenance of a diverse behavioral health and/or medical population. Population needs include both traditional psychotherapy and intervention with primary medical clients as needed. Occasional crisis intervention.

Essential Functions:

1. Maintains a detailed account of the clients’ needs, abilities and behaviors while working as a member of the behavioral health team.

2. Assess client’s needs, abilities, and behaviors using a variety of methods, including assessment, consultation with a multi-disciplinary team, and counseling.

3. Uses appropriate behavioral health techniques for the evaluation and treatment of clients.

4. Develops and implements intervention and treatment programs for clients.

5. Documents all services provided in Electronic Health Record (i.e., demographic data, treatment plans, psychosocial history, progress notes, discharge planning, etc.).

6. Talks and listens to clients who are suffering setbacks in their medical care due to psychological or behavioral issues.

7. Develops specific treatment plans or strategies that reflect the therapeutic intervention that addresses the client’s needs.

8. Designs and implements behavior modification programs as needed.

9. Provides culturally sensitive therapy modalities through existing services, outside referral services, or the development of new services.

10. Assists in clinical discharge planning, referring clients to, or providing alternative services.

11. Remains available for consultation and training to clinic primary providers and staff.

12. Provides reports/other utilization management documents to other providers or agencies involved in a client’s care.

13. Complies with CHC Standards of Conduct, Code of Conduct and Corporate Compliance.

Secondary Functions:

1. Use PPE as appropriate.

2. Understands role in Disaster Plan and that safety is a condition of employment.

3. Follows CHC’s policies and procedures.

4. Serves on various CHC and community committees as required/requested.

5. Attends meetings, seminars, and conferences as appropriate to ensure skills and knowledge remains current.

6. Other duties as assigned.

Requirements

Minimum Qualifications:

1. Ability to perform each essential function duty satisfactorily with or without reasonable accommodation. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

2. Licensed in the state of Arizona as a Clinical Social Worker or Psychologist

3. CPR certified (BLS).

4. Possess a valid driver’s license. 

5. Ability to read, write and speak English. 

6. Good interpersonal and human relations skills are required. Must possess skills to empathize and care for a variety of patients, including low-income and diverse cultures and treat everyone (internal and external customers) with respect and dignity.

7. Ability to maintain confidentiality at all times in all situations and follow the laws and intent of the most current HIPAA laws and regulations and Canyonlands’ security and privacy policies. 

Preferred Qualifications:

1. One (1) year of experience providing behavioral health, psychological or counseling services. 
2. Experience providing services in a medical setting using the integrated behavioral health model.
3. Bi-lingual (English/Spanish/Navajo) language skills
4. Knowledge of Microsoft programs with the ability to effectively communicate using Outlook.
