Job Summary:
Under the direction of the Executive Director, performs the following duties
as follows:
Duties and Responsibilities:
1. Conducts intakes and assessments of client needs.
2. Provides crisis intervention services to tenants in need.
3. Develops case management plans with clients to overcome personal
problems which resulted in, or contributed to, homeless status.
4. Refers clients to appropriate community resources.
5. Documents client progress in client files.
6. Advocates for client by contacting agencies providing requested
services.
7. Participates in case consultation with clinical staff as needed.
8. Ensures tenant compliance with project requirements and case
management plans.
9. Participates on organization and community committees as assigned.
10. Maintains patient confidentiality.
11. Performs other duties as assigned or as needed.
