About the Role
As a Family Coach in our Family Hope Center you will interview individual clients and families in order to assess their physical, social, emotional and spiritual needs. Provide case management services to Family Hope Center clients and assist individuals and families with issues such as poverty, drug addiction, child and spousal abuse, and homelessness. Organize support groups, help clients obtain community services, refer clients to other community agencies, and collaborate with civic, religious, and business groups to combat social problems through community programs.

What You'll Do
As a member of our team you will have an impact on our clients through accomplishment of the following responsibilities:

  • Participate as a member of the Family Hope Center team and assist with program development and implementation to meet the physical, emotional, social, and spiritual needs of clients/families as outlined in each client's plan of service.
  • Assist with the design and coordination of supportive services for families in accordance with program goals and objectives.
  • Timely and accurately compile and analyze intake and referral information to make an assessment of clients/families' needs; complete the enrollment process. Timely and accurately complete all necessary enrollment and assessment paperwork.
  • Conduct follow up interviews with clients/families at 30 days or as otherwise indicated by the program and each client's plan of service.
  • Provide case management services including, but not limited to, reviewing, evaluating, and updating individualized service plans, case notes, financial distributions, and referrals.
  • Coordinate services for group and individual therapy for assigned families/clients as appropriate. Provide counseling for crisis intervention as appropriate.
  • Assist program participants with accessing community resources, including, but not limited to, assisting with program enrollments and other services.
  • Develop professional relationships with institutions and agencies that provide education, job training, living skills training, employment services, housing, and child care services in order to coordinate services for program participants.
  • Analyze program needs; develop, modify, and implement changes in the program to meet changing needs of clients.
  • Demonstrate operational competence in efficiently managing resources.
  • Create and manage schedule of events and classes. Coordinate and facilitate meetings and trainings for clients and groups, including, but not limited to, Substance Abuse, Parent Education, Conflict Resolution, and Anger Management.
  • Develop activities and exercises for group and client education as appropriate. Appropriately interact with clients during program activities when necessary.
  • Prepare chart audits and documentation required for Continuous Quality Improvement, including satisfaction surveys, utilization reports, and follow up reports. Maintain accurate client information for the program.
  • Submit program reports regarding service delivery to designated personnel as required.
  • Liaison to schools and community and speak on behalf of Buckner to promote the program. Represent Buckner to outside agencies and their representatives.
  • Assist and support parents and guardians in working with the schools. Facilitate client's interaction with parents and school personnel as required.
  • Timely and accurately respond to inquiries regarding Family Hope Center program, detail eligibility requirements, maintain inquiry list, facilitate tours for prospective clients, as well as other interested parties, etc.
  • Timely and accurately maintain electronic and paper filing systems in accordance with established guidelines. Timely and accurately enter data for program participants into the CMS system.
  • Assist in publicizing the Family Hope Center program and educating the public about services provided by participating in presentations to the general public.
  • Serve as an advocate for the clients.
  • Work with community volunteers on special events to provide continued financial support to Buckner ministries.
  • Report suspected child abuse/neglect to the Director and appropriate law enforcement agencies.
  • Work nights and weekends as needed to meet business needs, including, but not limited to, trainings, special events, meetings, and emergencies.
  • Consult with supervisor on specific needs of clients.
  • Maintain compliance with all Buckner policies, procedures and requirements. Maintain compliance with all state and federal laws and regulatory requirements.
  • Work successfully as part of a team and responsible for developing then maintaining strong working relationships with co-workers.
  • Attend and participate in staff meetings and training as required. Attend and participate in in-service training, including CPR/First Aid and other clinical related training as required.

What You'll Bring to the Team
To be successful in this role and a great addition to our team we need you to come with the following:

  • A Bachelor's Degree in Social Work or a related field of study;
  • Minimum of 2 years prior related case work experience delivering services to youth and/or young adults and families;
  • Requires proficient working knowledge of and/or work experience using community resources.
  • Requires proficient ability to speak, read and write both English and Spanish.
  • Requires excellent public speaking skills and the ability to successfully and effectively communicate in a professional manner with a multi-ethnic population base in the community; ability to effectively communicate both orally and in writing.
  • Requires personality attuned to meeting the needs of the clients and ability to establish and maintain effective working relationships with other employees and the public.
  • Requires ability to speak clearly and make self understood effectively in face to face interactions; articulate with accuracy to speak on the phone.
  • Requires ability to hear and receive verbal instructions, answer phones, communicate with people in situations with some background noise.
  • Requires ability to maintain confidentiality.
  • Requires ability to effectively work under pressure and remain flexible as priorities change.
  • Requires ability to work under minimal supervision with awareness that error may have serious consequences; requires ability to exercise patience, initiative, and sound professional judgment.
  • Requires ability to work nights and weekends as required to meet business needs and provide services to clients.
  • Requires ability to understand and relate to the theories behind several related concepts; ability to remember verbal and written tasks/assignments from a few hours to long periods of time (months).
  • Requires proficient working knowledge and ability to accurately and timely operate and perform computer related tasks with specific equipment and software applications, including, but not limited to, word processing and spread sheets.
  • Requires professional commitment to Christian principles and teachings so as to perform tasks and responsibilities in alignment with Buckner's mission, vision, and values.
  • Requires ability to travel as required using various modes of transportation; ability to be on call as assigned. Requires ability to provide personal transportation for transporting clients and for other necessary travel to provide services to clients.
  • Requires ability to drive assigned vehicle(s) or personal vehicle, with appropriate state license, following all laws applicable; must provide proof of liability insurance and must be eligible to be insured under Buckner's insurance policy. Must be age 21 or older to drive on behalf of Buckner.
  • Requires ability to use up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people.
  • Requires ability to walk, stand and sit, sometimes for prolonged periods of time; ability to grasp, push, pull, carry or otherwise manipulate objects; occasionally required to squat, climb stairs, kneel and twist.
  • Requires sufficient good health to properly discharge duties. Employees shall not be permitted to work who have infectious disease or skin lesion for the duration of the communicability.

Preferred Qualifications
In addition to the amazing qualifications and experiences listed above, if we had our say, we would also look for:

  • Master's Degree in Social Work preferred in order to provide required supervision to program interns;
  • Ability to speak, read, and write Spanish to serve Spanish speaking clients in the community;
  • CPR/First Aid certification;
