SUMMARY

The Clinical Liaison will be responsible for promoting Broward Childrens Centers (BCC) programs throughout the medical community and assessing/coordinating patient readiness and transition into one of BCCs Medical Programs.

This position is required to operate a motor vehicle for business purposes and therefore, the Clinical Liaison must have a clean driving record (i.e. no more than 3 moving violations in the past 3 years; no arrests or convictions for DUI or DWI) and have and maintain valid automobile insurance.

EDUCATION and/or EXPERIENCE

Bachelor's degree, associate's degree, or nursing certification. Ventilator experience is required.

LICENSING

Valid and current Florida RN License

Valid and current Florida Drivers License

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

• Travel locally and regionally to promote BCCs Medical Programs; promote understanding of the medical factors related to BCC client/resident population.
• Travel locally and regionally an average of three (3) days per week, to assess and transition clients successfully to BCC; overnight travel as needed.
• Perform and coordinate the initial assessments; assess patients readiness to transition to one of BCCs Medical Programs and manage transitions of care.
• Direct patient care to Medically Fragile/Technologically dependent children and young adults during the discharge/intake process.
• Build relationships with the local and regional hospitals, rehabilitation centers, etc.
• Documentation, data entry and utilization reporting
• Participate in care plan meetings
• Ensure that the patient's medical needs are addressed; consult with the patients physicians as needed, coordinating plans of treatment, and advocating for the patient when necessary
• Identify and assist patient in accessing entitlements, resources, information, and referral
• Participate in Quality Assurance and Utilization review activities
• Other duties as assigned

LANGUAGE SKILLS and MATHEMATICAL SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

COMPUTER SKILLS

To perform this job successfully, an individual should have knowledge of Database software; Internet software; Spreadsheet software, Word Processing software and Presentation software.
