Breaking Ground is Hiring a Clinical Coordinator Near Queens, NY

Reporting to the Assistant Program Director, the Clinical Coordinator is responsible for ensuring that the service provision is of the highest quality and is consistent with housing first and harm reduction approach. The Clinical Coordinator oversees a staff of 4-6, liaises with psychiatric support services, interacts daily with multiple outreach providers, and serves as a member of the Program’s management team. This position's hours are flexible and will require some evenings and weekends. ESSENTIAL DUTIES & RESPONSIBILITIES: Provide clinical direction and leadership to the program; Help troubleshoot, problem-solve, and strategize solutions around client engagement and challenging behaviors Manage staff and promote a staff and resident culture that emphasizes housing placement as a measurement of success; ensure that clients are ready for housing placement within 90 days of entering the Drop-In Center Oversee development & implementation of individual service plans; ensure documentation of client contact and progress is comprehensive and timely Assess and evaluate client functioning Supervise and conduct case conferences Coordinate delivery of care with multiple service providers, particularly outreach teams Work collaboratively with outreach teams to ensure individuals succeed in the drop-in center/transitional housing while transitioning to transitional or permanent housing and avoid a return to homelessness Participate as part of the program management team to provide guidance and support to front-line staff, including security and house managers, in managing difficult situations and successfully de-escalating conflict Performs other related duties as assigned MINIMUM QUALIFICATIONS: Master’s Degree in Social Work and an active LMSW license or MPH degree is required Minimum 3 years’ experience working with chronically homeless populations Housing based case management experience strongly preferred Previous supervisory experience and strong clinical and assessment skills Proficiency in motivational interviewing techniques preferred Superb collaborative skills, ability to work successfully with a wide range of internal and external stakeholders Strong problem-solving skills Demonstrated success in working in a fast-paced environment Ability to communicate (verbally and written) with diverse populations and stakeholders Strongly preferred: Valid NY driver’s license with a driving record in good standing and maintaining a good driving record (as defined by the organization)Bilingual Spanish/English is preferred Consistent with NYC’s mandate, all Breaking Ground employees, interns, volunteers and subcontractors are required to be in receipt of an FDA authorized and/or approved COVID-19 vaccination and provide a one-time verification of immunization as a condition of employment.
