Borrego Health is Hiring an Early Intervention Specialist Near Riverside, CA

Summary:The Early Intervention Specialist willprovide HIV community outreach, screening and early intervention services to high-riskcommunities. To ensure that newlydiagnosed individuals are connected into care, those in care are receivingcomprehensive services and assist those PLWHA to reengage in care. Tocoordinate referrals and facilitate linkage to medical, dental, mental healthand other support services. Provide health education on various topics relatedto HIV/AIDS and conduct EIS assessments of patient needs to attain ongoingmedication compliance, continuity of care and zero viral load. This positionwill facilitate direct and immediate care coordination by ongoing communicationwith the established and potential patients. Essential Duties and Responsibilities: Outreach and HIV Testing and Counseling:1. As scheduled, conduct outreach, perform HIVtesting, conduct follow-up to ensure positive cases are linked to medical care andservices. Identify individuals out of care and assist with the transition oflinkage into care. 2. Build strong relationships with communitypartners in order to collaborate with them to determine what the unmet needsare in the service area.3. Assist in identifying new outreach sites andstrategies in the Coachella Valley, Hemet, and San Jacinto region of RiversideCounty.4. Conduct in-reach to ensure continuity of careand reengagement. 5. Assist in the development and implementation ofstrategies to find, link, and retain in care PLWHA. 6. Work diligently to identify individuals that are“out of care” and to ensure that once those “out of care” individuals areidentified, they assist them with the transition of being linked to care.7. Provide counseling, testing and linkage servicesto assist patients with assessing their risks, being tested, becoming aware oftheir status, and then getting linked into services. 8. Assist in building strong relationships withcommunity partners to collaborate on addressing the needs of PLWHA. Serve as a liaison in the community for EISprogram support.9. Arrange or participate in community events, presentations,and meetings and develop new partnerships with agencies serving as potentialentry points to HIV services. Linkage to Care and CareManagement1. Conduct EIS needs assessment with patients.2. Provide health literacy training to potentialclients so that they are better able to equip themselves with the necessarytools to identify and understand what barriers to care they may be facing.3. Conduct care coordination for medical, dental,and mental health services for all patients enrolled in the EHE programincluding assistance with appointment scheduling and hospital follow-up care. Monitor patient ongoing utilization ofservices and conduct appropriate interventions to encourage utilization.4. Assist patients who need additional resourcesvia interagency referrals to address needs and advocate resolving barriers inobtaining care and treatment adherence. Establish a rapport with other agencies to assist in obtaining services.5. Through ongoing communication with patient, confirmthat identified services were received and assist with resolving any identifiedbarriers. Maintain contact at least monthly with patients in the program.6. Link clients to medical care by scheduling adoctor appointment, ensuring that the client gets to that appointment, and byfollowing up on subsequent appointments with the doctor.7. Maintain accurate records for referrals andservices provided.8. Ensure timely entry of services delivered intoappropriate database (ARIES). Administrative1. Process and manage care coordination inadherence to established program policies, procedures and workflow. Address thelevel of urgency in accordance with the identified need.2. Trackcaseload and other related case referral reports to ensure timely tracking of patientsvisits and if applicable, results are obtained in a timely manner to movethrough the continuum of care.3. Dataentry of all program specific module, reporting dashboard and EHR system.4. Assist with monthly/quarterly/annual servicereports of EIS activities including data, referrals, and service provisions andservice units. 5. Ensure timely entry of services delivered intoARIES.10. Participates inHIV/AIDS and program related training and education to advance skills/knowledgeof HIV and related disease process, medications, and treatments.11. Participates in quality assurance activities.12. Demonstrates cultural sensitivity andcompetency.13. Performsall other duties and activities as the assigned. Qualifications: Minimum Qualifications:1. Threeyears plus in HIV programmatic experience and/or healthcare program development2. HIVTesting and Counselor Certification.3. Experienceworking with priority populations, 50 HIV positive, including injection drugusers, as well as Hispanic, African American and the LGBTQ populations.4. Knowledge of community services,social service support agencies,and resources for patients.5. Experience and knowledge of HIV/AIDS, and sexuallytransmitted infection patient education.6. Sensitivity to AIDS, chronic illness, LGBTQ issues anda strong commitment to the mission, vision and values of Borrego Health areessential.7. Valid California Driver’s License Special Conditions of Employment:1. All staff that is presently working within aclinic is required to have a Current and Valid CPR and First Aide certificationon file with the Human Resources Department. This is for clinical andadministrative staff. This new license requirement is mandatory for continuedemployment, CPR and First Aide certification must be obtained through theAmerican Heart Association. Internet certificates do not comply and for thisreason are not valid. Knowledge, Skills and Abilities:1. Excellent oral and written communicationskills.2. Effective interpersonal skills. 3. Excellent customer services.4. Motivational Interviewing.5. Computer Skills.6. Bilingual in Spanish/English Preferred Physical and Mental Requirements: 1. Able to lift/move up to 40 pounds, move from place toplace, and stand for long periods.2. Ability to do math, organize and prioritize workload,work effectively and efficiently under stress.3. Ability to supervise, multitask, understand and followinstructions.4. Move throughout the clinics and community with theability to travel as needed. Customer Service:1. Actively supports, promotes, and works tofulfill the Mission, Vision and core values of BCHF.2. Provides excellent internal and externalcustomer service. 3. Demonstrates BCHF’s Standards of CustomerService Behavior: Compassion, Attitude, Communication, Appearance, Sense ofOwnership, and Teamwork. 4. Participates in on-going customer servicetrainings.5.
