Summary:The Local Community Base Outreach COVID-19 (LBCOC-19) Community Health Worker provides individual and community orientation and education to assist individuals in accessing and receiving COVID-19 vaccination. Conducts outreach programs in vulnerable and medically underserved communities including racial and ethnic minority groups. Promotes programs, events, and related activities. Provides orientation on other healthcare services. Assists in making appointments for COVID-19 vaccine and or testing. Provides information about public insurance programs and refers the uninsured to the CCS department for application assistance. As directed, participates in events, gives presentations, and conducts one-on-one orientation and education. Accurately completes and submits grant related COVID-19 vaccine surveys using an electronic tablet and assigned application. Provides counseling regarding vaccine concerns and misinformation. May assists in making appointments for individuals who wish to consult a medical provider to discuss concerns that could prevent them from receiving the COVID-19 vaccine. Essential Duties and Responsibilities:1. Collaborate with community partners to provide COVID-19 vaccine education and community awareness to increase vaccine confidence and reduce preconceived fears of the COVID-19 vaccination.2. Presents COVID-19 vaccine education and promotion – at community events, schools, churches, libraries, and others. May be required to conduct virtual educational activities. 3. Provides vaccine education program information to the public, such as educational brochures, flyers, and promotional materials. 4. Utilizes updated COVID-19 vaccine education libraries to provide resources for staff and patients.5. Documents activities, recording information such as the number of classes completed, presentations conducted, total number of attendees, total number of surveys completed, among others. 6. Utilizes databases, mailing lists, telephone networks, and other information to facilitate the functioning of COVID-19 vaccine education programs. 7. Participate in outreach events.8. Conducts face-to-face outreach and reaching out directly to community members to educate them about the COVID-19 vaccine. 9. Collaborates with other health agencies and or community agencies to promote COVID-19 vaccine education in the community. 10. Provides group educational sessions or one on one sessions as needed. 11. Conducts an annual estimate of 680 individual COVID-19 vaccine surveys using electronic tablet and assigned application and or electronic platform. 57 Individual surveys per month at minimum.12. Conduct case management and data entry for individuals who after a one on one or group session are determined and interested in receiving their COVID-19 vaccine.13. Provides monthly reports of appointments made, total number of surveys conducted and outreach efforts. 14. Works as a liaison between communities-based programs such as but not limited to Elementary Schools, Day Cares, Pre-schools, and High Schools, WIC sites, local churches, community centers and Borrego Clinics. 15. Other duties as assigned.Qualifications: Minimum Qualifications:1. Graduate of approved high school or GED equivalent2. Outreach experience preferred3. Community Health Education experienced preferred.4. All staff that is presently working within a clinic is required to have a Current and Valid CPR and First Aide certification on file with the Human Resources Department. This is for clinical and administrative staff. This new license requirement is mandatory for continued employment, CPR and First Aide certification must be obtained through the American Heart Association. Internet certificates do not comply and for this reason are not valid.5. Valid driver license, clean driving record, and reliable transportation.
