Summary

The Clinical Therapist provides a variety of therapeutic interventions, including counseling, and outreach/case management services to an assigned caseload. The Clinical Therapist is responsible for assessing each individual’s needs as they relate to their treatment and provide ongoing support.

Responsibilities

• Provide a variety of therapeutic interventions appropriate to the consumer’s needs;
• Conduct psycho-social assessments on clients to develop and implement a personalized treatment plan;
• Prepare transfer and closing summaries;
• Completes all required billing in a timely manner;
• Identify resources and appropriate interventions;
• Monitor clinical services to ensure achievement of treatment goals identified in treatment plan;
• Document all contacts accordingly;
• Maintain up to date case documentation within electronic health record (EHR) ;
• Attend regular team meetings, supervision meetings, and all other assigned meetings.
• Provide enhanced services using interventions with individualized outcomes focusing on reduction of symptoms related to both mental health and substance abuse;
• Provide consistent outreach (home visits, telephone, and letter) in accordance with agency program guidelines and maintain contact with significant collaterals;
• Monitor consumer medication compliance and facilitate scheduled doctor’s appointments;
• Assist consumers in applying for SSI and/or Medicaid immediately upon case assignment;
• Attend required staff meetings, agency meetings, weekly supervision and other meetings and events as required;
• Participate in ongoing professional development workshops and seminars as required by the organization and other governing bodies;
• Engage in independent study on an ongoing basis in the clinical areas applicable to consumers being served to include but not limited to study of the DSMIV, classifications and uses of psychotropic medications, the Mental Health Code, the Child Abuse Reporting Act, the confidentiality Act and Therapeutic Interventions;
• Enforce and maintain policies, procedures, regulations, and guidelines;
• Maintain a high level of confidentiality and ethical standards;
• Participate in conferences as needed with the consumer and psychiatrist regarding medication and treatment interventions;
• Maintain effective communication with staff, consumers, referral sources, etc.;

Qualifications and Education

• Master’s degree required in psychology, social work, counseling, or related field;
• Committed to providing high quality consumer care and service;
• Excellent time management skills;
• Experience in mental health counseling preferred;
• Proficiency using Electronic Health Records system (EHR);
• Ability to handle sensitive and confidential information.
