Position Overview:

The Intake Coordinator is a key role within Bluestone Physician Service’s Clinical Operations department. This role is responsible for day to day execution of all processes related to new patient enrollment, Legal paperwork retrieval, data documentation and customer onboarding. This position works closely with Physician Teams, Families, and Facility Staff to ensure successful onboarding.

Responsibilities:

  • Accurately and efficiently process enrollment on a timely basis.
  • Provide exceptional customer service to family members, facility staff, and Bluestone peers through multiple channels such as phone, e-mail, and written letters.
  • Gather and analyze a variety of documents including legal Health Care Power of Attorney documents. Provide accurate data entry into our electronic health record (EHR) systems and Portal platforms. Strictly adhere to regulatory compliance guidelines.
  • Educate patients and caregivers about legal health care decision-making documents, Bluestone requirements, and assist them in meeting these requirements.
  • Ability to multi-task and be flexible while adapting to the unique needs of each Bluestone Team and Facility Staff.
  • Ability to prioritize time and participate in special projects and duties as needed.
  • The ability to work as an interactive team member or independently.
  • Ensure all required patient documentation is complete and signed prior to processing.
  • Data entry of all patient demographics in electronic medical records and Bluestone Bridge.
  • Participate in special projects and other duties as assigned
  • Forward patient information and completed charts to appropriate physician team.
  • Provide exceptional customer service to family members, facility staff, provider teams, and Bluestone peers through multiple channels such as phone, e-mail, and written letters.
  • Communicate regularly with Community staff, family members, and Provider Teams to ensure highest level of customer service.

Qualifications:

Education/Certification/Experience

  • 2 years of experience working in the healthcare field in an administrative capacity.
  • Experience reviewing and analyzing legal documents highly preferred
  • Exposure to electronic health record (EHR) platforms preferred
  • Experience with medical insurance desired.
  • Comfortable navigating electronic medical records.
  • 2 years experience with and navigating Power of Attorney Documents

Knowledge/Skills/Abilities

  • Detail oriented and accurate.
  • Strong interpersonal and customer service abilities.
  • Skill in oral and written communication.
  • Intermediate level computer proficiency in word processing, spreadsheets, and database.
  • Ability to communicate professionally with supervisors, coworkers, customers, and patients.
  • Ability to work independently to meet company goals.
  • Demonstrated compatibility with Bluestone’s mission and operating philosophies.
  • Demonstrated ability to read, write, speak, and understand the English language.
  • Skill in identifying and resolving problems.
  • Ability to deal with change and ambiguity
