Overview

Position Summary: Coordinates the development and implementation of the Service Plans (SP’s) also called Individual Support Plans in PA or Essential Life Plans in DE for individuals supported in those respective programs.

Responsibilities

Essential Responsibilities: 1. Coordinates and evaluates the effective development and implementation of SP’s for people we support to contribute to success in achievement of their life goals. These plans will increase individual skill acquisition, and/or meet the specific habilitation needs of the individuals as well as decrease interfering behaviors: 2. In conjunction with each individual’s support team, complete annual assessments thoroughly and in accordance with agency and regulatory time frames, procedures, and established requirements. 3.Assist with identifying goals and service objectives, developing data collection systems to monitor established goals, and making improvements or modifications to all aspects of the SP. 4. Attends team meetings comprised of the people we support, their families/guardians/advocates, support staff, and external case management/supports coordinators, to both provide and obtain information regarding the SP, review baseline goals, identify individual strengths and weaknesses, evaluate progress of persons served regarding attainment of goals, determine the need for modifications to existing goals, combine goal teaching or rehabilitation and development strategies for best possible outcome, and discuss concerns regarding specific persons served to ensure overall SP effectiveness; 6. Creates and maintains master schedule for SPs to ensure they are completed and distributed in accordance with regulatory required time frames and procedures; o Coordinate, facilitate, and complete all aspects of the annual SP meetings in accordance with agency and regulatory requirements. Assure that the meeting and the process is person centered. o Actively lead the team process in conjunction with the SP for all individuals. Coordinates meeting logistics, takes meeting minutes, records action plans, and performs follow-up to facilitate the timely implementation of such plans. 

Qualifications

Position Requirements: Education & Experience: Minimum BA degree in Human Services or related field with a minimum of two years of work experience with individuals with intellectual or developmental disabilities required. Previous experience as a Supports Coordinator, Case Manager or Program Specialist is desirable. Previous experience working with HCSIS, Promise, Therap, and/or other CLS billing system is desired. Special Skills: Prior experience in providing clinical services to special populations with a special interest in teaching, reintroducing and/or rebuilding the functional skills necessary to increase the level of independence within residential, educational, vocational, and community-based settings. Ability to effectively communicate both verbally and in written form required. Basic math skills required. Effective interpersonal skills, a strong desire to work with neurologically-challenged individuals required, and the ability to implement crisis intervention techniques required. Effective time management, organization, and prioritization skills required. Operative knowledge of MS Office Word and Excel spreadsheets preferred. Attainment of First Aid, Crisis Prevention and Intervention, and Medication certification within first 90 days of employment required. Valid driver’s license required in incumbent’s legal name and current address with no provisional restrictions. Minimum 18 years of age required.
