Dedicated CNAs needed for PM and Night Shift (2p-10p, or 10p-6a) to care for our residents at Alden Terrace of McHenry. Certified Nurse Aide's help perform ADL's, perform direct care and other nursing functions at our post acute and long-term care facility. JOB SUMMARY

Provides residents with daily nursing care in accordance with current federal, state and local standards, guidelines and regulations, facility policies and as may be directed by the Charge Nurse, Supervisor, Assistant Director of Nursing, Director of Nursing or Administrator to ensure that the highest degree of quality care is maintained at all times.

SUPPORT DUTIES

  • Commit to continued growth as a knowledgeable and experienced healthcare provider in the area of post-acute care, custodial care, and Alzheimer’s dementia and other diseases that cause dementia.
  • Commit to obtaining at least 12 hours of continual education annually for self, including the required 16 hours of orientation training and any other ongoing dementia care specific trainings for direct care staff working on a memory care unit.
  • Become knowledgeable of each individual resident’s background, life history, disease, medical and behavioral care needs, and preferences in detail.
  • Utilize any communication devices for residents, such as language services, communication boards, assistive devices, etc. Accept education for utilizing these mechanisms to provide ample communication.
  • Assist in providing a therapeutic, person-centered care that constantly strives to enhance the quality of life of each resident.
  • Encourage family involvement through volunteering and visitation.
  • Provide a clean, safe, non-hazardous unit for the residents and staff.
  • Assist in providing an individualized symptom management tracking system as needed per resident, including development and maintenance of individualized, non-pharmacological interventions implemented.
  • Complete all required documentation.
  • Maintain all program policies and procedures and state and federal regulations.
  • Provide feedback to nurses for monitoring achievement of restorative goals through a team approach.
  • Be flexible to meet the change in residents’ needs.
  • Provide emotional, caring support to residents, families, and the inter-disciplinary team.
  • Perform other duties as assigned.
