Assisteo Home Health is looking for a Full-Time RN in the Scottsdale/North Phoenix area!

JOB DESCRIPTION SUMMARY

The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. Provides direct patient care as defined in the Arizona Nurse Practice Act.

2. Completes an initial assessment of patient to determine home care needs. Completes OASIS based on data obtained. Provides a complete physical assessment and history of current and previous illness(es). Performs a medication reconciliation on initial visit.

3. Regularly re-evaluates patient nursing needs.

4. Uses health assessment data to determine nursing diagnosis.

5. Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.

6. Develops a care plan, which establishes goals based on clinical diagnosis and incorporates therapeutic, preventive, and rehabilitative care.

7. Administers medications and treatments as prescribed by the physician, within the scope of practice.

8. Provides health care instructions and counsel to the patient and family as appropriate per assessment and plan of care.

9. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

10. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.

11. Communicates with physician regarding the patient’s needs and reports any changes in patient’s condition; obtains/receives physician’s orders as required to coordinate plan of care.

12. Ensures protection of confidential information that is stored, handled, or maintained by the Agency per HIPAA regulations.

13. Submits required documentation in an accurate and timely manner within Agency guidelines from time of visit.

14. Understands and practices all State and Federal Laws within the scope of practice.

ADDITIONAL DUTIES

1. Participates in on-call duties as defined by the on-call policy (see Policy 2-032.1).

2. Ensures that arrangements for equipment and other necessary items and services are available for patient.

3. Uses medical equipment, PPE and supplies effectively and efficiently.

4. Instructs, supervises, and evaluates home health aide care provided every two (2) weeks and Licensed Practical Nurse every 30 days, at the minimum.

5. Must participate in agency’s sponsored in-service training.

6. Participates in personal and professional growth and development.

POSITION QUALIFICATIONS

1. Registered Nurse with current license to practice professional nursing in Arizona or qualifying compact state.

2. Graduate of accredited school of nursing.

3. Two years’ experience is preferred, at least one of which is in the area of public health or home care nursing.

4. Must have a valid driver’s license and able to work in the United States.

5. Must have working knowledge of Miscrosoft Office Suite and ability to learn to use Electronic Medical Records Systems.

6. Demonstrates excellent observation, problem solving, organizational, verbal and written communication skills along with the ability to handle multiple tasks using sound judgement and available resources with minimal supervision.

7. Meets and/or exceeds skill requirements per competency checklist.

8. Maintain current CPR certification.

9. Maintain current TB test results.

10. Maintain current fingerprint clearance card with the State.

11. Maintain current auto insurance in accordance with State and/or Agency requirements.

12. Maintain good standing with the Department of Motor Vehicles.

13. Automobile must stay in good working order and is a reliable mode of transportation.

14. Must be able to pass a background check for pre-employment and ongoing checks per Agency guidelines.

15. Must be COVID-19 Vaccinated or have a qualifying exemption and agree to Agency tracking process.

WORKING ENVIRONMENT/PHYSICAL REQUIREMENTS

Prolonged or considerable walking, standing, driving. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity, hearing, dexterity, and attention to detail to perform required nursing skills. Must have ability to perform CPR.
