Direct/Indirect patient care
ESSENTIAL FUNCTIONS
The Licensed Practical Nurse is a licensed professional who provides direct and indirect patient care services delivered on a visiting and intermittent basis in the patient’s home and in accordance with the policies and procedures of the Agency. All care provided by the LPN is supervised by a Registered Nurse, who will make regularly scheduled supervisory visits with the LPN to document care is being given per physician orders.
EDUCATION/EXPERIENCE REQUIREMENTS
· Must have one or more years in home health
· Enteral feedings, Wound Vac, is preferable
· Must have a working knowledge of computers
· Current CPR certification
· Current TB test
· Current Finger print card
· Must have valid Driver’s License
· Must have reliable vehicle throughout employment, may not use public transportation
REQUIRED COMPETENCIES – KNOWLEDGE, SKILLS, ABILITIES
· Furnishes skilled nursing services to the patients in their places of residence, using the plan of care created by the RN
· Prepares clinical and progress notes for care provided
· Assists the physician and RN in performing specialized procedures
· Prepares equipment and materials for treatments observing sterile and aseptic technique as required
· Assists the patient in learning appropriate self-care techniques
· Ensures protection of confidential information that is stored, handled or maintained by the Agency per HIPAA regulations
Maintaining confidentiality of patient and agency matters Submitting required documentation in a timely manner – within 48 hours from time of visit
Knowledgeable of electronic record usage
Must be licensed in the State of Arizona as a Licensed Practical Nurse
WORKING ENVIRONMENT/PHYSICAL REQUIREMENTS
Physical effort involving above-average standing, sitting, walking, detail work and driving.
ENVIRONMENTAL CONDITIONS – HAZARD RISK
Frequently – Exposure to blood, body tissues, and other potentially infectious fluids
Frequently – Exposure to other hazardous waste materials
Frequently – Exposure to unpleasant odors as related to care of incontinent residents or wounds
Frequently – Exposure to latex, plastic and/or materials which are used for personal protective equipment
Frequently – Exposure to marked changes in temperature and/or humidity
Frequently – Exposure to microbial bacteria and other infectious agents inherent to care of ill patient
