- Complete initial and on-going interdisciplinary assessment of the patient with an emphasis on level of functioning. Also completes OASIS assessments at appropriate time points.
- Execute assessment using procedures such as manual and electrical muscle examinations, functional evaluations, gait analysis, sensory testing, measurements of range of motion, and assessment of development and utilization of human movement.
- Develop and evaluate of the plan of care in partnership with the patient, representative (if any), and caregiver(s), and in consultation with the physician and interdisciplinary care team members.
- Provide services that are ordered by the physician as indicated in the plan of care.
- Provide therapeutic treatments and evaluation of equipment needs, as applicable.
- Current, active license as a Physical Therapist by the Physical Therapy Examining Committee of the California Board of Medical Quality Assurance
- Core competency per skills checklist
- At least one year of experience in physical therapy.
