The primary function is to provide personal healthcare and related services to the client in their place of residence; assist in providing a safe and clean environment, work cooperatively with patient and family, and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.
- Assist clients with personal care and hygiene.
- Help clients with physical therapy exercises, if required.
- Remind clients to take prescribed medication.
- Be a pleasant and supportive companion.
- Report any unusual incidents.
- Act quickly and responsibly in cases of emergency.
- Education: High School Diploma preferred.
- Licensure: Must have current driver's license, Insurance, and own vehicle.
- Experience: At least one year experience in the last 24 months as a Home Health Aide or Nursing Assistant in a Hospital, nursing home, or home health agency, or completion of a Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
- Others: Reliable transportation. Valid and current auto liability insurance.
- Home Health.

- Evening shift.
