Responsibilities

To greater meet the needs of our individual and corporate clients, the Ascension St. Vincent  Hillenbrand Family Wellness Clinic in Batesville, Indiana is seeking a  part-time (.5 FTE) Family Medicine APP  to focus on optimizing patients’ overall health by assessing their current health risks, and providing chronic disease management, disease prevention, education and detection.  The duties will also include medical care with disease management, working with a team of physicians to deliver high quality comprehensive care to patients with a focus on relationship building as well as also offering same day appointments for acute illness.

Requirements

Licensure/Certification/Registration:

Required:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• One or more of the following required:
  • Nurse Practitioner credentialed from the American Nurses Credentialing Center (ANCC) obtained within 6 Months (180 days) of hire date or job transfer date.Valid state license required specific to the state in which the associate works.
  • Nurse Practitioner credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date.Valid state license required specific to the state in which the associate works.
  • Nurse Practitioner credentialed from the National Certification Corporation (NCC) obtained within 6 Months (180 days) of hire date or job transfer date.Valid state license required specific to the state in which the associate works.
  • Registered Nurse credentialed from the American Nurses Credentialing Center (ANCC) obtained prior to hire date or job transfer date.Valid state license required specific to the state in which the associate works.
  • Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date.Valid state license required specific to the state in which the associate works.

Education:

• Master's degree of Nursing required.
