Reason for need: Vacancy - Personal Emergency Start Date: April 21 End Date: May 31 (dates could be added, if still open and provider desires) Dates Needed: April 21 7am to April 25 7am AND May 22 7am to May 31 7am NPDB self-query and case logs must be submitted Board Certification required, with active AZ license and DEA Call Only (Beeper) Response requirement: 15 minutes; Phone consults required Patients: 7-9; Admissions: 6 or less EMR: Athena Temporary Privileges are available Patients per shift: 7 - 10 Department beds: 12 Estimated Credentialing Timeline: 2-3 days Travel and Lodging provided Malpractice Insurance provided
