Ascend Community Staffing Partners LLC is Hiring a Family Support Specialist Near Rome, GA

Description

Ascend partners with Highland Rivers Behavioral Health, Georgia’s largest Community Service Board (CSB), to staff clinical and non-clinical jobs in behavioral health. Being part of our team and working in a CSB means you’re helping individuals and families otherwise unable to access treatment from private providers to address healthcare challenges associated with mental illness, intellectual/developmental disabilities, and addictive diseases. You’ll be working with purpose to make a difference in the lives of people who need you most. Each of our jobs will work within a State Community Service Board facility. 

  • Complete intakes for Family Support Program referrals, providing orientation to families and explanation of services/funding
  • Write comprehensive Individual Family Support Plans based on individual’s needs
  • Enter new individuals in electronic health record
  • Scan documents into electronic health record
  • Assist other Family Support Coordinator with monitoring and servicing of 182 individuals
  • Travel as needed across 12 counties and to out of town provider meetings or trainings
  • Attend meetings on individuals and community resource fairs, or home visits as requested
  • Answer phones and provide quality customer service
  • Assist families with other resources and networking as needed, complete surveys
  • Process Family Support vouchers
  • Complete filing and assure accuracy of charts
  • Work with or assist in transition to other programs with regional office staff, support coordinators, providers and other community agencies
  • Any additional duties assigned by supervisor

High School diploma required 

College preferred 

6 months experienced with Intellectual Developmental Disabilities

Must be able to pass an intensive criminal background investigation
