Ascend Community Staffing Partners LLC is Hiring a Community Support Specialist Near Dalton, GA

Description

Ascend partners with Highland Rivers Behavioral Health, Georgia’s largest Community Service Board (CSB), to staff clinical and non-clinical jobs in behavioral health. Being part of our team and working in a CSB means you’re helping individuals and families otherwise unable to access treatment from private providers to address healthcare challenges associated with mental illness, intellectual/developmental disabilities, and addictive diseases. You’ll be working with purpose to make a difference in the lives of people who need you most. Each of our jobs will work within a State Community Service Board facility

  • Assist individuals with service needs, developing strategies and supportive, social support networks and coordinating rehabilitation services as specified in the consumer’s individual recovery plan.
  • Partner with the individual and CORE services provider in the development of the individual service plan.
  • Assist the individual with the transitioning between levels of care. Link and/or serve as advocate with other agencies to garner needed resources for consumers.
  • Provides assistance and support in crisis situations
  • Provides assessment, monitoring, and assistance to individuals.
  • Establishes professional and ethical relationships with individuals, families, and coworkers.
  • Provides direct service to individuals; completes all necessary paperwork according to policy; ensures individual meet eligibility criteria or refers to appropriate provider in the community.
  • Manages a caseload by ensuring all services are authorized, chart paperwork is complete and current, and consumer charts are closed according to Policy & Procedure.
  • Sees a caseload at the frequency defined in the service plan; documents attempts to contact individuals and ISP reviews; completes monthly caseload reports; track appointments and case management

This position is a Pay for Performance position, which requires that staff who work 40 hours per week complete 35 hours of billable services per pay period. 17.5 per week of billable services (Pro-rated for those who work less than 40 hrs per week)

Requirements

Bachelor’s Degree in helping field

At least one year’s experience working in mental health or substance use disorder field preferred.

Must be able to pass background investigation, drug screen and acceptable motor vehicle report.
