Description

Summary: Responsible for initiation and completion of the necessary procedural steps for establishment and implementation of direct provision of all supported living services for individuals assigned to CES Medicaid Waiver services. Responsible for coordinating and monitoring individual service plans, services delivered, and progress; responsible for hiring, training, supervision/discipline, payroll, evaluation of Direct Service Provider (DSP).

Essential Duties and Responsibilities

  1. Conducts a preliminary assessment for new referrals.
  2. In coordination with assigned Case Manager, assists with developing a plan of service, participates in continued stay reviews and personal future plans.
  3. Determines direct care service delivery staffing needs and arranges for the delivery of services through conducting interviews, hiring, supervising, producing payroll payments, evaluating, and discipline of DSP.
  4. Participates in team meetings with family members, professionals, and case managers to implement procedures and programs that ensure regulation compliance.
  5. Coordinates schedules for both waiver and generic services for client that includes staffing of all alternative living services.
  6. Performs Medicaid billing payroll functions for DSP on a bi-weekly basis and assures the integrity of that billing by approving time, attendance and time cards, reviews and approves DSP travel, and documents work performed.
  7. Monitors all in-home services on a regular basis that includes ensuring Direct Care Worker compliance with completion of progress notes, required paperwork, code of ethics, and meeting agency requirements for initial and continued employment.
  8. Ensures DSP is compliant in following positive behavior plans and completing reports as required.
  9. Completes in a timely and accurate manner monthly, quarterly, and annual reports and various other reports as determined by Supervisor.
  10. Provides training to new DSP in general procedures and individual specific training.
  11. Reviews DSP progress notes, inputs data into client workbook monthly, and creates quarterly reports that focus on recipient's progress in meeting goals and objectives.
  12. Counsels recipients and families regarding staffing issues, scheduling, and other issues under their scope of responsibilities, and is on call to assist with staffing emergencies.
  13. Responsible for monitoring client medication regimens, ensures staff is at all times aware of medications being used; monitors consumption of medications on at least a monthly basis; ensures screenings are performed at a frequency determined by the prescribing physician, and any additional requirements as determined by DDS rules and regulations. The Direct Care Supervisor will serve as a liaison between staff and the Physician regarding medication.
  14. Prepares appropriate paperwork for new plans, continued stay reviews and revisions in accordance with DDS standards.
  15. Works with Case Management entity to assist and act as liaison for recipients with professional services to include, but not limited to early intervention coordinators, doctors, public schools, case management agencies, counseling services, Human Services, nursing homes, retirement centers, and other DDS agencies.
  16. Serves as the individual's advocate in assuring access to a wide range of services. Documents services that are needed by or recommended for the individual but do not exist within the individual's local community and works to develop the availability of these services.
  17. Assures that all personal rights of the individual are maintained, including freedom of provider choice, civil, and legal rights.
  18. Perform any other related duties as required or assigned.
  19. Complete monthly report and include the following: staffing issues, new hires, training dates, focus groups, monthly/quarterly visits, incident reports, payroll issues and/or resolutions to issues

Requirements

Qualifications Requirements:

To perform this job successfully, an individual must be able to perform each essential duty competently. The requirements listed below are representative of the minimum levels of knowledge, skill and/or ability required. Requirements may be modified to accommodate individuals with disabilities. Must possess Arkansas driver’s license, have current liability auto insurance, and be physically able to drive in various locations in Arkansas.

Education and/or Experience:

A Bachelor’s Degree in Human Services or a related field plus a minimum of 2 years’ experience working with individuals with developmental disabilities or a related field. Six (6) years’ experience working with individuals with a developmental disability or in a related field may be substituted for education.

Language Skills:

Ability to read and understand Agency and other related regulations. Ability to coordinate, organize and file relevant documents. Ability to communicate effectively with individuals, co-workers, parents, regulatory agencies and the general public.
