Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

• In collaboration with physician, you will provide and coordinate primary care services to an assigned panel of Advantage members who reside in the community
• You will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives
• You will also act as clinical resource to the participating providers and facilities
• You will act as team member and clinical resource to participating providers/facilities and to provide resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process
