Description of Position:
Staff is responsible for assisting residents (adults 18 years or older) with medication and for ensuring that clients are safe and housing is in good condition.

Who We Are: Conexio Care, an affiliate of Apis Services, Founded in 1985 as a subsidiary of the organization then known as Church Home Foundation, Conexio’s original mission was to help older adults with lifelong histories of psychiatric hospitalization to transition into the community. Our mission expanded in 1988 to include homeless services and substance abuse treatment.
Education and Experience:
- High School Diploma.
- Valid driver’s license in good standing with at least 3 years minimum continuous driving experience.
Professional Credentials/Certifications:
- CPR/First Aid (Certification required).
- MANDT Training.
- Assistance with Self-Administration of Medication (Certification required).
- Conexio Care All Staff Orientation Curriculum.
Responsibilities:
- Assists in the planning, and preparation of medication routes, as required.
- Assists clients with medication administration as necessary.
- Develops relationships with all clients and encourages all clients to become involved in the community and as independent as possible while providing necessary support.
- Reports crisis situations or potential crisis situations to supervisor and to appropriate treatment provider as directed by supervisor. Reports suspected incidents of neglect, abuse, or mistreatment as per agency policy.
- Provides crisis intervention with clients as needed.
- Assists with in-office tasks as assigned by supervisor.
- Complete Medication Administration Record as required by law.
- Maintains client confidentiality as per agency policy.
- Maintains accurate Conexio Care car logs.
- Must adhere to Conexio Care Policy regarding usage of Conexio Care Vehicles.
- Assists in maintaining the safety of clients.
- Adheres to internal agency policy.
- Performs other duties as requested or assigned, verbally or in writing.
