• Conduct treatment interventions and activities including assisting clients with daily living tasks and social skills, carrying out or assisting with recreational activities, observing and reinforcing behaviors as prescribed by the treatment plan, and intervening to resolve conflicts as they arise.
• Provide personal care assistance to residents in the areas of personal grooming, bathing, dressing and undressing, oral hygiene, hair care, shaving, nail care, nutrition and hydration, toileting, housekeeping, and self-administration of medication.
• Monitors the well-being and location of clients, completing documentation in the shift log. Makes notes to the clinical record as assigned.
• Ensures health, safety, and well-being are maintained by adhering to internal health and safety policies and PM 46 regulations.
• Attends and participates in daily shift reports, staff meetings, and training sessions as required by policy and/or as assigned.
• Observe and document any hazardous conditions and health and safety issues as they occur.
• Document and report changes in building conditions /client’s mental and physical health to the Team Leader and the Core Team.
• Assist others in emergencies, ensuring that the Team Leader is alerted to the problem and is involved in the decision-making process in an unusual event.
• Know/carry out emergency procedures including fire safety procedures and first aid.

• High School Diploma or GED.
• Completion of accredited CNA program.

• Delaware certification as a certified nursing assistant.
• CPR and First Aid Certification.
• Valid driver's license and clean MVR. (motor vehicle record)
