We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.
The Patient Care Coordinator works with Patient Care team, the Business Office, Medical Records, and the Clinical Management team to coordinate and assure updated patient information in the electronic medical record, copies of necessary forms, and assessment of services due to all patients.
