AFA History: AFA was founded in 2002 by a family caregiver to ensure families affected by Alzheimer’s disease always had a place to turn for guidance, support, and services.

AFA Mission Statement:The mission of the Alzheimer’s Foundation of America (AFA) is to provide support, services and education to individuals, families and caregivers affected by Alzheimer’s disease and related dementias nationwide, and fund research for better treatment and a cure.

Position Summary: The Social Worker will assist and provide supportive feedback and resources to individuals and families as they are faced with Alzheimer’s and Dementia related issues via phone calls, chats and emails. Overall Helpline PT shifts include:

Monday - Friday 5:00pm-9:00pm

Saturday – Sunday 9:00am-5:00pm

Saturday – Sunday 5:00pm-9:00pm

Responsibilities:

• Provide counseling, educational information and resource referrals to clients via phone calls, emails, chats and conversations.

• Link callers and their families to appropriate services and programs.
• Assess and address safety and/or risk and develop safety and intervention plans accordingly.
• Facilitate support groups over the phone for caregivers, family members, and individuals with dementia.
• Assist with special projects and other duties as assigned by the Director of Educational and Social Services.
• Must be flexible and available for weekend shifts.

Qualifications:

• Master’s Degree in Social Work or related field (Psychology, Counseling, Mental Health or Gerontology) and NY State Licensed.
• A minimum of 1-year relevant work experience with the elderly population, caregivers and health professionals in the field of dementia and Alzheimer’s care.
• Excellent interpersonal, listening, verbal and written communication skills.
• Excellent and continuous follow up skills a must.
• Computer proficiency in MS Office and email correspondence required.

Preferred:

• Bi-lingual Spanish (not required), flexible work schedule evenings/weekend day.
