Altus Hospice is currently seeking a Hospice Intake Coordinator to join our team in Corpus Christi. If you are looking for a career with a purpose and a supportive team culture, then this is the position for you!
The Hospice Intake Coordinator is responsible for all aspects of the patient intake and admission processes including directing and coordinating and initiation of hospice services, and any other equipment, supplies, external services for hospice clients as prescribed by the physician, and/or requested by the client/family. Hospice Intake Coordinators respond to all incoming and outgoing physician and payer communication (insurance verification and authorization), and are in frequent communication with hospice clinical staff. Additionally, the Hospice Intake Coordinator assists the Admissions Nurse by following the patient throughout the referral-intake-admission process to facilitate a smooth transition to the hospice program.
