Overview

St. Luke’s Health System in Meridian is seeking a Nurse Navigator to join our Spine Care Clinic team.

The Nurse Navigator is a patient advocate. They will triage incoming referrals and patient calls. This requires general knowledge of spine wellness and the ability to recognize potential red flag symptoms. This role requires critical thinking and independent decision making skills. The Nurse Navigator will work closely with providers and medical assistance in patient care coordination for Treasure Valley Clinics (Meridian and Nampa).

Monday - Friday, 8:30AM - 5:00PM.

Responsibilities

The Nurse Navigator offers medical guidance, acts as a support group, and guides patients and their families through the treatment process.

• Serves as a single point of contact for patients undergoing treatment,.
• Serves as a liaison between the patient and their family and the patient's network of care providers.
• Educates patient on diagnosis, treatment options, and prognosis.
• Assists the patient in scheduling appointments, completing paperwork, or securing referrals.
• Facilitates short term and/or long term interventions.
• Possesses comprehensive knowledge of subject matter.
• Handles complex issues and problems, and refers only the most complex issues to higher-level staff.
• Other duties and responsibilities as assigned.

Qualifications

• Education: Bachelors of Nursing Degree
• Experience: 2 years' of RN experience
• Licenses/Certifications: Current RN License in the State of Practice
