All Care Homecare is seeking to hire a full-time & part-time Home Care LPN to provide support for clients with disabilities. All Care Homecare is a participant in the Group Adult Foster Care Program (GAFC). As part of this program, we provide personalized, culturally sensitive, compassionate care to seniors and individuals with disabilities. Collaborating with treatment providers, we establish a comprehensive individualized care plan for each of our clients. Responsibilities:

  • deliver professional nursing services in client's home setting and provide case management for primary patients
  • complete initial assessment and plan of care for home care clients
  • prepare clinical and progress notes, submitting all required paperwork daily
  • coordinate services, informing the physician, management, and other personnel of changes in the client's condition and needs
  • ensure quality and accuracy of patient records and charts in compliance with Medicare and Medicaid standards
  • demonstrate compassion while working with ill, disabled, elderly and/or emotionally disturbed patients
  • respond promptly to telephone calls and emails
  • participate in continuing education programs to stay current on home health practices, laws and regulations
  • post hospital visits
  • monthly client visits
  • help to maintain confidentiality files
  • medication management services
  • perform other duties as assigned

Education and Experience Requirements:

  • RN with current MA license, BSN preferred
  • one or more years of current nursing experience in certified home care is preferred, but not required
  • ability to sit, stand, bend, lift, kneel, climb stairs and move patients safely as required
  • ability to work in the patient's home environment as well as the office
  • demonstrate strong verbal, written and interpersonal skills to communicate effectively
  • must have reliable automobile, valid driver's license, and current auto insurance
  • fluent in English with skills in a second language a plus
  • computer skills sufficient to capture notes and required documentation
