General Description of Duties and Responsibilities:
The Program Coordinator is responsible for assisting the residential program’s back end operations. The ideal candidate is a quick study, organized, good communicator, good follow through and follow-up. This role requires a person that's flexible and able to work well with multiple people, employees, participants and departments. Must be willing to travel between sites and pick up necessary items for sites.
Specific Duties:
- Assist with completing and renewal applications (SNAP and MA).
- Assist with the participants' waiver renewal process.
- Review appointment medical paperwork after submission and update the tracking board.
- Assist with chart reviews and MAR audits.
- Assist with filing and also purging documents.
- Manage site key system.
- Review site maintenance requests, submit and follow-up with the maintenance department..
- Put together packets for site team meetings.
- Keep the chart and filing neat and organized.
- Work with all departments to ensure communication flows between departments.
- Assist with identifying free/low cost activities for participants.
- Assist with the preparation of lunch and learn meetings (office and sites).
- Place calls to sites to announce fire drills and review fire drill binders quarterly for accuracy.
- Reconcile petty cash forms after the P.S. review purchases for accuracy.
- Assist with chart reviews.
- Ensure sites have necessary posting, supplies, etc.
- Ensure all vehicles are put into the fleet by assigning number, sticker, posting and clip board with necessary items inside.
- Print updated ISP every quarter for site and update the ISP in the participant binder.
- Print updated BSP and RPP for site binder.
- Assist with the preparation of new site development, postings and binders.
- Handle non-supervisory calls from sites.
- Point of contact for Community Hab referrals.
- Accompany staff during meet and greet for new admissions.
- Act as a backup for the front desk.
- Assist with administration tasks when needed.
- Other duties as assigned.
Qualifications:
The minimum professional qualifications for Program Coordinator are an Associate’s Degree in Human Services and/or High School Diploma and 3 years of relative experience.
- Good computer skills.
- Good organizational skills.
- Word, excel and PowerPoint.
- Must be 21 years old or older.
- Autonomy – ability to work without immediate supervision.
- Interpersonal sensitivity (objective awareness of others, tactful interaction, ease in establishing limits).
- Reliability and flexibility
- Insight and the ability to articulate it.
- Valid driver’s license and the ability to drive a van.
- CPR, First Aid and AED.
- Must clear background checks.
