Advir Health Care Systems LLC is Hiring a Direct Care Staff Near Locust Grove, GA

Part time Days and Eveningsmust be able to work weekends
Job Duties: The Direct Care Staff works under the direct supervision of an assigned supervisor and is responsible for the direct care of individuals, such as Elderly, Medical Compromised, Mentally Disabled and Children receiving services from the agency. This job is for an individual.
Qualifications:
  • Must be able to read and write.
  • First Aid/CPRDCH/NCIC/DBHDD Background clearance.
  • Direct Support Professional experience is preferred, but not required.Capable of lifting 50 pounds without assistance.
  • Maintains current driver’s license if doing transportation.
  • Must have a positive attitude, flexibility, and ability to work with people of diverse backgrounds.
Duties and Responsibilities:
  • Must notify supervisors 4 to 24 hours if calling off via of phone no texting.Do not call the client must notify Advir; who will call the client.
  • Supports individuals in attaining personal goals and desires.
  • Ensures safety and security of individuals.
  • Turns and re-positions bedridden individuals, alone or with assistance, to prevent bedsores.
  • Completes all progress notes and documentation by the end of shift or before change of shift.
  • Assist individuals with ADLs.
  • Routine hygiene care throughout the day.
  • Supervise in addressing their ISP/Plan of Care goals and document progress.
  • Support all opportunities for the growth, learning and development of the individual.
  • Complete daily progress notes highlighting person center events.
  • Complete understanding of the agency’s policies and procedures.
  • Attend all staff meetingsMaintain individuals’ records on site in accordance with policy and procedures.
  • Attend mandated trainings as required.
  • Assumes full responsibility when at work for food preparation, community activities, and assisting individuals with other needs.
  • Supports individuals by planning nutritious meals, and supporting those with special diets.
  • Cooperates with family members regarding reasonable requests related to the needs of individua.
  • Supervises the self – administration of prescribed medication and maintains record of medication by documentation on MARs according to established policies (if applicable.
  • Maintains medical chart for each individual, consistent with physician’s orders, and reports any medical emergencies according to the Medical Emergency Plan.
  • Administer medications to those individuals unable to self-administer (must be proxy trained).
  • Assumes responsibility for cash disbursements given by supervisor and keeps receipts showing actual amount spent, ( if Applicable).
  • Supports individuals with learning and maintaining personal and household skills.
  • Provides physical supports to residents; including, but not limited to, lifting and moving adaptive equipment
  • Receives visitors to the facility; maintains family oriented nature of the facility by being hospitable to family members and other natural supports by making arrangements as needed for family if applicable.
  • Supports and encourages individuals through use of positive reinforcement and non-intrusive consequences.
  • Abstains from mental, physical or verbal abuse, humiliation, withholding personal funds, and the use of physical restraint or isolation.
Other Requirements:
  • Transportation (only if the standards required by DBHDD are met.
  • EDWP no transportation is required.
  • Do not give the Client your phone number.
  • Work without direct supervision.
  • Participates in personal work evaluationsMaintains a good attendance record.
  • Reports for work on time, observes policies regarding hours worked and punctuality.
  • Works as needed, according to personnel policies.
  • Maintains a helping relationship with other staff and Board of Directors.
  • Represents Advir Health Care Systems in a positive way to the general public.
  • Maintains the flexibility to change routines, if needed.
  • Possesses the mental and physical ability and stamina to perform the job well.
  • Follows all other requirements of current personnel policies.
  • Performs other reasonable related duties as assigned by the supervisor.
  • All Training required by DBHDD, will be met in accordance with the standards. Competency.
  • Fluent in the written and spoken English language.
  • Basic computer skill.
  • Able to Read and Write.
  • Able to use computer.
  • Trainings as required by DBHDD, EDWP.
  • Ongoing training as directed by Supervisor.
