Advantage is currently seeking a Per-Diem/PRN Medical Social Worker to join our vastly growing Hospice team in the Harrisburg, PA and Surrounding Areas! This position is responsible for providing patient care in their home.

Responsibilities:
- To function as a resource to patients and families in accessing community services to meet their socioeconomic needs.
- To ensure quality and safe delivery of social services.
- To ensure that social services, reflecting the Agency's mission and standards, are provided in accordance with federal, state, and local standards, guidelines, and regulations.
- To demonstrate support for achieving the mission of the Agency.
Qualifications:
- Masters degree of social work accredited by the Council on Social Work Education
